📋 Tanggung Jawab Pekerjaan

• Coordinate patient care for asthma patients, including scheduling appointments, monitoring treatment plans, and ensuring patients receive the best possible care.
• Collect and analyze survey data to improve patient outcomes and identify areas for improvement in patient care.
• Work closely with healthcare providers to ensure patients receive the best possible care and to address any issues or concerns that may arise.
• Stay up-to-date with the latest developments in asthma care and patient management.
• Provide regular reports and updates to healthcare providers and other stakeholders on patient care and survey data.
• Participate in team meetings and other professional development opportunities to stay current with industry best practices.
• Ensure all patient care and survey data are accurately recorded and reported in accordance with industry standards and regulations.
• Maintain a high level of confidentiality and professionalism in all interactions with patients, healthcare providers, and other stakeholders.

🎓 Kualifikasi & Syarat

• Bachelor's degree in healthcare, nursing, or a related field.
• Minimum of 2 years of experience in healthcare coordination or a related field.
• Strong knowledge of asthma care and patient management.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficiency in using Microsoft Office Suite (Word, Excel, PowerPoint).
• Familiarity with healthcare software and electronic health records (EHR) systems.
• Valid driver's license and reliable transportation.
